I agree to release from responsibility and waive any claims that may arise against the foundation or any foundation personnel relative to my attending the retreat and participating in the program and any activities.
“The information that we collect from you on this form includes your personal information. Your personal information is protected by law, including by the Commonwealth Privacy Act. The client management system that we are using is an IT system called the ‘Data Exchange’. This system is hosted by the Australian Government Department of Social Services. Your personal information that is stored by the Department on the Data Exchange will only be disclosed to us for the purposes of managing your case. The Department de-identifies and aggregates data in the Data Exchange to produce information for policy development, grants program administration, and research and evaluation purposes. This includes producing reports for sharing with service providers. This information will not include information that identifies you, or information that can be used to re-identify you, in any way. You can find more information about the way the Department will manage your personal information in the Department’s APP privacy policy, which the Department has published on its website. This policy contains information about how you may access the personal information about you that is stored on the Data Exchange and seek correction of that information. This policy also includes information about how you may complain about a breach of the Australian Privacy Principles by the Department, and how the Department will deal with your complaint.”

To provide effective and high-quality services to Carer’s and to maintain appropriate accountability, The Carers Foundation must collect and store relevant personal information about individuals who access our service. We respect the confidentiality of information you provide and take the privacy of every individual seriously.
What type of personal information does the Carers Foundation collect?
We may ask for a range of personal information to assist us in providing you with the most relevant and up to date service provision and care. This includes (but is not limited to) names (for you and the people or person you care for), addresses, dates of birth, contact details and employment details. The Carers Foundation may request sensitive
information, this includes information relating to a person’s religious beliefs or affiliations, racial or ethnic origin and health and medical history. The Australian Privacy Principles set out restrictions about the way sensitive information can be used. Unless you give us your consent, or if we are required or permitted by law, we will only use or disclose sensitive information for the purposes for which it was provided.
How does The Carers Foundation collect personal information?
The Carers Foundation will collect your personal information directly from you where this is reasonable and practical. This may be during face-to-face meetings, interviews, via registration/feedback forms or telephone calls. We may also collect personal information from fundraising events; third parties – for example, from referring organisations or your parents or guardians; and via our website/facebook page including if you use it to contact us, make a donation or give us feedback.
How does The Carers Foundation use personal information?
The Carers Foundation will only use your personal information for the purpose for which it was provided for. Such purposes include: advising of upcoming training or retreats, providing information about events; news or publications that may be of interest to you; for the purpose of research and program development; general administration such as internal reporting; as required or permitted by law.
How does The Carers Foundation store personal information?
We keep personal information securely. We do not share the personal information of The Carers Foundation service users with other staff or volunteers unless it is necessary for service provision. We do not share the personal information of The Carers Foundation service users with any outside person or body, unless we are legally authorised to do so.
Accessing and correcting information:
You may request to access or correct the personal information we hold about you; or make a privacy related complaint. We ask that if you are aware of any information we hold that needs to be updated or corrected you please let us know as soon as possible and you can contact us if you wish to opt out of receiving newsletters and other communications.
